Summary
This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval – award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.
Former EDRP participants are ineligible.
- Fully qualified and certified to function in a clinical role in accordance with the guidelines and policies of the CBOC and the WNCVAHCS.
- Delegated authority and responsibility to assume major clinical, consultative, educational, research, and administrative duties related to the examination of claimants filing compensation and pension claims.
- Independently manages a panel of patients with complex chronic‑care issues using evidence‑based national guidelines to achieve optimal population health and effective disease management.
- Leads and collaborates with the Primary Care Team and other interdisciplinary teams to identify, analyze, and coordinate resources to resolve care issues and meet patients’ medical and psychosocial needs.
- Appropriately orders diagnostic and therapeutic tests relative to a patient’s age‑specific needs and health condition.
- Prescribes pharmacologic and non‑pharmacologic interventions that support the patient’s ability to maintain or regain health status.
- Monitors and documents the effectiveness of treatment interventions and achievement of institutional quality measures and performance metrics.
- Initiates referrals or consults to the appropriate service when indicated.
- Sets clinical care goals—short and long term—in collaboration with the patient, provider(s), and family members or caregivers.
- Provides education to patients and families regarding complex medical, social, and behavioral conditions with a focus on self‑management, prevention, and wellness based on the patient’s health goals and needs.

- Works closely with the immediate supervisor and nurse manager to complete evaluations in a timely, detailed, and quality‑focused manner.
- Assists with arranging continuity of care across the VHA system as needed.
- Provides on‑site leadership to ensure care is completed within required time‑frames.
