Job Title: Community-Based Physical Therapist (PT) | Help Patients Thrive at Home

Compensation: $100,000–$200,000 annually + $8,000 Retention Bonus

Schedule: Full-Time | Monday–Friday

• Evaluate patients in their homes and develop individualized treatment plans.
• Improve mobility, strength, balance, endurance, and overall functional independence.
• Educate patients and caregivers on exercises, adaptive strategies, and injury prevention.
• Collaborate closely with nurses, physicians, occupational therapists, speech therapists, and the interdisciplinary care team.
• Monitor patient progress, modify treatment plans, and document outcomes accurately.
• Empower patients to remain safe, independent, and confident at home.

• Active Minnesota Physical Therapist (PT) license.
• Strong clinical reasoning and excellent communication skills.
• Passion for helping patients maximize independence in their own homes.
• Ability to work independently while managing a field-based schedule.
• Valid driver's license, reliable transportation, and willingness to travel throughout the Twin Cities Metro.
