Physical Therapist – Home Care – Full-Time

Schedule:

Full‑time, Monday to Friday, 8:00 am – 4:30 pm (flexible hours).

Location:

Home‑care patients in Stillwater, MN;
Washington County, MN; and St. Croix County, WI. Pay rate:
Up to $78 per hour depending on experience; mileage included. This assignment is temporary with an open‑ended vacancy; candidates interested in direct hire or temp‑to‑hire will also be considered.

• Assess the physical, psychological, social, and environmental needs of the patient and family.
• Integrate principles of development and disease processes, including comorbidities, in creating the plan of care.
• Maintain necessary records and use them appropriately for patient care.
• Provide patient and family education, including self‑care and care provided by family members.
• Serve as a case manager for assigned patients and coordinate with all providers involved in patient care, including RN, SW, home health aide, and other rehabilitation disciplines.
• Assist patients and families in resolving concerns or complaints of care or service, and provide direction and supervision of care by home health aides.
• Assess needs and refer to Adult Protection Services regarding the status of vulnerable adults as necessary.
• Provide skilled clinical care, including evaluation and treatment of patients, and development/implementation of care plans.
• Anticipate the needs of the population served and the unique characteristics of the communities served.
• Perform all other duties as assigned.

• Current license with the Minnesota Board of Physical Therapy and either a current or obtainable Wisconsin license.
• Bachelor’s, master’s, or doctoral degree in physical therapy.
• At least one year of prior experience as a PT in home‑care settings.
• Knowledge of age‑related growth and development principles to provide appropriate service for primary diagnoses and multiple comorbidities affecting care.
• Proficiency with communication and data‑processing equipment, clinical apparatus related to home care, and electronic medical record systems.
• BLS certification through the American Heart Association (AHA) or American Red Cross (ARC).
• Reliable transportation for client visits.
