Job Title:

  • Complete comprehensive Physical Therapy evaluations in patients' homes
  • Develop individualized plans of care and functional goals

  • Help reduce fall risk and improve home safety
  • Provide therapeutic exercise and functional mobility training
  • Educate patients and caregivers on home exercise programs and safe mobility techniques
  • Recommend strategies and equipment to improve independence
  • Monitor patient progress and adjust treatment plans as needed
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary clinicians
