Position OverviewAs a Licensed Practical Nurse (LPN) with Superior Hospice, you'll provide compassionate, hands‑on care to patients in their homes, assisted living communities, memory care settings, and skilled nursing facilities throughout the Twin Cities Metro.
This is an excellent opportunity for an LPN who enjoys building meaningful relationships while practicing independently in the field. As our organization continues to grow, this role offers the opportunity to expand into a full‑time position.
What You'll Do- Provide skilled nursing care including symptom management, medication administration, wound care, and ongoing patient assessments.
- Monitor patient conditions and communicate changes to the Registered Nurse and interdisciplinary team.
- Educate patients, caregivers, and family members regarding hospice services and individualized care plans.
- Deliver compassionate support during each stage of the hospice journey.
- Complete timely and accurate EMR documentation.
- Participate in weekend, on‑call, and holiday rotations to ensure exceptional patient care.
- Active Minnesota Licensed Practical Nurse (LPN) license.
- Previous experience in hospice, palliative care, home health, skilled nursing, or long‑term care required.
- Excellent communication, organization, and interpersonal skills.
- Ability to work independently while collaborating closely with an interdisciplinary care team.
- Compassionate, patient‑centered approach with strong clinical judgment.
- Reliable transportation and a valid driver's license.
