Common Bond Communities is seeking a Service Coordinator II to provide independent service coordination for residents, including those in permanent supportive housing, and to promote community building and engagement.
You will conduct strengths-based assessments, develop resident-centered plans, and coordinate with property management partners while maintaining accurate records. A collaborative, client-focused approach and CPR/First Aid certification are preferred.
