We are seeking a driven and relationship-focused Home Health Marketer / Sales Liaison to expand our presence in the Fort Worth/Terrell market and surrounding communities.
Position SummaryThe Sales Liaison will be responsible for developing and maintaining referral relationships with physicians, hospitals, SNFs, ALFs, and community partners. This role is ideal for a motivated, goal-oriented professional who thrives in a fast-paced environment and is passionate about healthcare sales.
Key Responsibilities- Build and maintain strong referral relationships within assigned territory (Forney,/Terrell, TX and surrounding areas)
- Generate qualified home health admissions (Medicare/Episodic and Non-Episodic)
- Conduct daily field visits to physicians, hospitals, case managers, and community partners
- Track and manage all activities through CRM (daily call logs required)
- Collaborate with clinical and intake teams to ensure smooth patient transitions
- Meet and exceed monthly admission targets and performance goals
- Represent the agency at networking events and community outreach initiatives
- Base Salary:

- Minimum 1–2 years of experience in home health, healthcare marketing/sales (preferred)
- Strong understanding of Medicare and home health guidelines and referral processes
- Established relationships in Forney/Terrell area (highly preferred)
- Excellent communication, presentation, and relationship-building skills
- Self-motivated with a proven track record of meeting or exceeding sales goals
- Reliable transportation and willingness to travel within territory
- A go-getter who thrives on hitting targets and driving growth
- Someone who can build trust quickly with referral sources
- A professional who is organized, accountable, and results-driven
