Position Registered Nurse

• Makes the initial and/or comprehensive nursing evaluation visit, determines the primary focus of care, and develops the plan of care within state-specific guidelines.
• Conducts home visits to clients in designated geographic territories.
• Initiates, updates, and implements the plan of care in collaboration with physicians and other health care professionals.
• Communicates clients’ current health status and needs to appropriate staff.
• Provides on‑call coverage and responds to calls.
• Documents and completes POC for errors and findings.
• Completes initial assessments and aide assignment sheets for client personal/attendant care.
• Ensures patients meet home‑health eligibility and medical‑necessity guidelines as defined by payer source.
• Audits and reviews medication logs.

• Current RN licensure in state of practice.
• Current CPR certification (required).
• Three years of registered nurse experience (preferred).
• Current driver license and vehicle insurance.
• Must be 18 years of age or older.
• Must pass a TB skin test and drug screen.
• Must pass a criminal history check.
