Deliver care and services for designated group of guests/residents according to Individualized Service Plans (ISP);
Report all incidents immediately to appropriate supervisor / team member;
Participate in the care planning for their designated guests/residents and perform individualized personal care and activities of daily living (ADL);
Obtain, document and report the guest/residents vital signs, urine output and meal intake per individualized care plan to licensed nurse as requested;
Observe and report any physical and/or psychosocial changes of condition requiring a licensed nurse intervention and care services
