Position: Hospital at Home Coordinator
A company is looking for a Hospital at Home Coordinator (Remote - Evening & Weekend Hours).

Key Responsibilities Manage patient intake and service coordination for the Hospital at Home program Schedule skilled and non-skilled visits and verify completed services Communicate with clinicians and internal teams to ensure continuity of care Qualifications Experience in home health care, scheduling, intake, and care coordination Proficiency with healthcare software systems (Kinnser and/or Alaya Care preferred) Ability to work independently in a remote environment Reliable internet connection and dedicated workspace Experience supporting hospital-at-home or acute home care programs is preferred
