Hospice Nurse On Call

Hours: Wednesday/Thursday 4pm-12am Friday/Saturday 8pm-8am

Territory: Fort Collins

Position Profile

On‑call RN’s at Bloom hospice are primarily responsible for providing and coordinating hospice care for patients and families during after‑hours. They collaborate with the interdisciplinary hospice team to ensure patients’ physical, emotional, and spiritual needs are met. They educate patients and families about the disease process, help manage and control symptoms, and provide support during difficult times. Care delivery occurs in the patient’s home or community location.

Responsibilities
  • Provide hospice patient care after‑hours as needs arise.
  • Provide effective pain/symptom assessment and management.
  • Provide effective patient/family/caregiver teaching.
  • Manage end‑of‑life ethical issues effectively.
  • Create timely and accurate documentation.
  • Utilize resources effectively and efficiently.
  • Adhere to scope of practice and continuously improve processes and services.
  • Maintain and enhance professional skills.
  • Adhere to high standards of personal and professional conduct.
Minimum Qualifications
  • Graduate of accredited school of nursing.
  • Current CO RN licensure in good standing.
  • Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.
  • Strong clinical assessment skills.
  • Excellent communication and interpersonal skills.
  • Solution driven, creative and resourceful problem‑solving skills.
  • Proficient computer skills.
Preferred Qualifications
  • Previous hospice or oncology experience.
  • Knowledge of the death/dying and bereavement process.
  • CHPN certification.
