Responsibilities- Review clinic schedule and identify patients appropriate and due for Medicare Annual Wellness Visits (AWV).
- Work collaboratively with clinical teams to accomplish AWV efficiently for patient and team.
- Follow AWV protocols and standing orders, including completion of preventive services resulting from AWV.
- Develop and share knowledge of AWV eligibility and documentation requirements.
- Develop knowledge of Medicare preventive services and clinical guideline recommendations.
- Appropriately document within the electronic record to satisfy quality gaps.
- Assist with other clinical duties such as prior authorizations, patient messages, and prescription refills.
- Utilize appropriate protocols and standing orders in delivering patient care.
- Participate in regular team meetings, huddles, staff meetings, and quality improvement projects.
- Consult with clinical staff and ancillary department staff to eliminate barriers to efficient delivery of care.
- Identify service delivery problems and potential for effective patient management intervention.
- Work independently and exercise clinical judgment in interactions with physicians, payers, patients, and families.
- Document all communication with patients in the electronic medical record.
- Maintain strict confidentiality and adhere to HIPAA guidelines and regulations.
- Travel to other CPH locations as needed.
- Perform other duties as assigned.
- Background checks will be performed with an offer of employment.
- Current RN license.
- Knowledge of medical model specific to Annual Wellness Exams.
- Knowledge of patient assessment techniques, including taking medical histories, performing physicals, and evaluating health status.
- Knowledge of using medical equipment and instruments to administer patient care.
- Knowledge of medical records and documentation in a confidential manner.
- Ability to make responsible decisions within scope of practice.
- Ability to collaborate effectively with physicians.
- Ability to maintain quality control standards.
- Ability to react calmly and effectively in emergency situations.
- Ability to interpret, adapt, and apply guidelines and procedures.
- Ability to communicate clearly and establish/maintain effective working relationships with patients, medical staff, and the public.
- Medical Translation Spanish Certification (CHI or CMI) is valued and considered for additional compensation.
- Successful completion of an approved RN program.
- 1–3 years of experience performing Annual Wellness Visits in a clinical setting.
- Position is required in‑person.
Physical Requirements
- Varied activities including standing, walking, reaching, bending, and lifting.
- Full range of body motion including handling and lifting patients, manual and finger dexterity, and eye–hand coordination.
- Must be able to use a variety of office equipment.
- May require working under stressful conditions.
- Normal office and exam room environment.
- Ability to work under pressure and with a diverse population, including staff, physicians, clients, patients, insurance companies, labs, hospitals, and other members of the public.
- May cause exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinical environment.
