Job OverviewLocation:
Montrose, CO. Salary: $40.07 – $48.10 per hour (based on prior experience).
Schedule:
PRN.

- Graduate of an accredited school of professional nursing program;
Baccalaureate degree preferred. - Current RN license in Colorado.
- Minimum 2 years of nursing experience in long‑term care, dementia care, or acute care. Home‑care experience preferred.
- CPR certified.
- Provides case management, coordination, supervision, and implementation of professional and supportive services to clients using nursing theory and process in accordance with the Nurse Practice Act.
- Reviews client referral information and responds to requests and inquiries as appropriate.
- Performs client assessments based on agency standards of practice to ensure effective and appropriate home care services.
- Directs, plans, and initiates appropriate action independently and responsively in in‑home care situations.
- Performs job duties in accordance with agency policies, procedures, and professional and community standards.
- Performs other job duties as assigned by the Director of Clinical Services.
Home Health of Western Colorado is a Medicare/Medicaid‑certified home health care agency in Montrose, CO. Our experienced team of nurses, rehab therapists and home health aides are dedicated to providing individualized health care needs with compassion and empathy. We are trained in caring for older adults and provide in‑home health care services in the Delta, Montrose and Ouray counties.
