Montrose, CO. Salary: $40.07 – $48.10 per hour (based on prior experience).

• Graduate of an accredited school of professional nursing program;
  Baccalaureate degree preferred.
• Current RN license in Colorado.
• Minimum 2 years of nursing experience in long‑term care, dementia care, or acute care. Home‑care experience preferred.
• CPR certified.

• Provides case management, coordination, supervision, and implementation of professional and supportive services to clients using nursing theory and process in accordance with the Nurse Practice Act.
• Reviews client referral information and responds to requests and inquiries as appropriate.
• Performs client assessments based on agency standards of practice to ensure effective and appropriate home care services.
• Directs, plans, and initiates appropriate action independently and responsively in in‑home care situations.
• Performs job duties in accordance with agency policies, procedures, and professional and community standards.
• Performs other job duties as assigned by the Director of Clinical Services.

Home Health of Western Colorado is a Medicare/Medicaid‑certified home health care agency in Montrose, CO.
