Position Overview:
Boone Heart & Wellness in Ft. Collins, Colorado, is seeking a Nurse Practitioner or Physician Assistant with experience in hormone optimization/replacement to join our successful, growing wellness and lifestyle medicine practice. Boone Heart & Wellness provides high level, state of the art preventive medical care in a small scale and intimate setting where we get to know our patients.
The ideal candidate will work collaboratively with physicians and other healthcare professionals to provide comprehensive, patient-centered care. You will play a critical role in managing patients with multiple health concerns, using evidence-based methods to proactively identify, prevent and treat the most common chronic conditions that adversely affect health and wellbeing.
Key Responsibilities:
- Schedule would be 3 to 4 days per week that includes patient care, 90-minute new patient visits, annual wellness check ins, 30-60 minute follow up visits and telemedicine appointments. No on-call or weekends.
- Conduct thorough patient assessments, including medical histories, physical exams, imaging studies and lab evaluations, to identify cardiovascular risks, metabolic dysfunction, nutrient insufficiencies, hormone imbalances and other health concerns.
- Develop and implement personalized treatment plans based on the principles of preventive and functional medicine, focusing on root causes and addressing lifestyle factors that impact overall health.
- When appropriate, be able to provide medical management and pharmaceutical interventions for common chronic conditions including cardiovascular diseases, metabolic dysfunction and obesity.
- Evaluate and treat functional gastrointestinal concerns and understand the basic role of the GI tract in other chronic conditions.
- As part of a comprehensive care plan, identify, treat and monitor hormone imbalances related to menopause, andropause, thyroid dysfunction, and other related conditions.
- Educate patients on lifestyle modifications, nutrition, supplementation, stress management, hormone optimization and other wellness strategies to improve overall wellbeing and health span.
- Order, interpret, and follow up on laboratory tests and diagnostic imaging to assess and monitor patient conditions.
- Stay current on the latest research and advancements in preventive, lifestyle and functional medicine to provide the highest level of care.
- Collaborate with other healthcare professionals, including physicians, nutritionists, wellness coaches, and physical trainers to provide comprehensive care.
- Maintain accurate and up-to-date patient records in accordance with healthcare regulations and practice guidelines.
- Be an exceptional educator and collaborator with your patients so they understand treatment options, potential side effects of medications/hormones prescribed, and the importance of lifestyle changes in achieving optimal wellness.
