Wellness Nurse

Morning Star Senior Management LLC is seeking a compassionate Wellness Nurse (LPN or RN) to support residents in our Old Town senior living community. In this on-site role, you’ll provide high-quality clinical care, administer medications, monitor resident health, and support individualized care plans across assisted living and memory care. Collaborate with caregivers, families, and providers in a faith-inspired, servant-leadership environment that honors seniors.

• Provide direct nursing care and wellness support to senior residents in assisted living and memory care
• Administer medications safely and accurately per physician orders and community policies
• Monitor, document, and report changes in resident condition using EHR systems
• Implement and support individualized care plans in collaboration with interdisciplinary team
• Communicate effectively with residents, families, and healthcare providers
• Promote infection control, fall prevention, and safety protocols throughout the community
• Support, mentor, and collaborate with caregiving staff to ensure quality care and service
• Participate in training, audits, and quality improvement initiatives to enhance resident outcomes
