Arizona Mental Health & Recovery (AMHR) is seeking a dedicated full‑time Case Manager for its Ascent First Episode Psychosis team. In this hands‑on role you will deliver comprehensive, client‑centered case management services that connect individuals and their families to essential resources and supports—housing, medical care, benefits, community services—within a collaborative, transdisciplinary team that offers up to five years of wraparound care.

- Serve as payee for clients who need assistance managing their finances.
- Help clients meet basic needs for food, clothing, shelter, personal safety, and general health care, and assist with applications for income, medical, housing, or other benefits to which they are entitled.
- Ensure clients’ access to needed services and community resources by arranging transportation.
- Provide comprehensive psychosocial services designed to improve or maintain clients’ abilities to function effectively.
- Help clients evaluate strengths and symptoms, and facilitate goal setting and planning for appropriate services.
- Guide and instruct clients in daily living skills such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, and use of transportation.
- Help clients develop social skills, interests, and leisure activities, including age‑appropriate opportunities.
- Help clients find and leverage employment, vocational rehabilitation services, or supported work environments as appropriate.
- Offer support, assistance, consultation, and education to families, friends, landlords, employers, community agencies, and other contacts to maximize benefits and minimize community complications.
- Identify and work with natural support systems such as neighborhood networks, churches, and community organizations to encourage treatment program engagement.
- Reach out to eligible clients, inform them of, and educate them about available community services.
- Bachelor’s degree in psychology, social work, or a related human services field.
- OR Bachelor’s degree in an unrelated field with at least one year of behavioral health experience.
- Valid Colorado Driver’s License with a driving record that meets organizational requirements.
- Demonstrated interest in the behavioral health field.
- Ability to assess crisis situations and intervene appropriately.
- Knowledge of community resources.
- Basic understanding of and ability to work with differing behavioral health presentations.
- Effective organizational skills.
- Effective written and verbal communication skills.
- Preferred:
Bilingual.
