CES (Coordinated Entry System) Specialist

The CES (Coordinated Entry System) Specialist reports to the CoC Director and is responsible for overseeing the administration, coordination, and continuous improvement of the Coordinated Entry System (CES) within the Northern Colorado Continuum of Care (No

CO CoC). This role ensures an efficient and equitable housing placement process by collaborating with community partners, refining system policies and procedures, and facilitating training initiatives.

The CES Specialist leads coordination of Coordinated Entry activities across the CoC, including facilitation of the CAHPS Steering Committee and communication with partner agencies participating in the by-name list process. The CES Specialist is responsible for supporting the day-to-day functioning of the Coordinated Entry process, ensuring assessments, prioritization, referrals, and partner participation occur consistently across the CoC.

Additionally, the CES Specialist works closely with the HMIS team to support data-informed evaluation efforts that strengthen system performance, equity, and access for vulnerable populations.

The CES Specialist works closely with CoC staff and community partners to ensure Coordinated Entry operations align with CoC priorities, grant requirements, and HUD Coordinated Entry guidance.

This position requires strong facilitation, analytical skills, and the ability to foster collaboration among diverse stakeholders.

• Data Management & Analytics
• Manage Coordinated Entry System (CAHPS) data for individuals experiencing homelessness in Northern Colorado using HMIS and Vela.
• Create and analyze custom reports in HMIS using Looker software to monitor CAHPS data quality, enrollments, outcomes, and system performance.
• Integrate and maintain a confidential, de-identified client list of victims of domestic violence using CAHPS data by working with local victim service providers.
• Support preparation of Coordinated Entry data and metrics for grant reporting, system performance monitoring, and other CoC data requests.

• Collaboration & Facilitation
• Convene and facilitate Coordinated Entry case conferencing meetings with CAHPS partners to coordinate identification, assessment, prioritization, referral, and housing navigation for households on the by-name list, while working collaboratively with partners to address barriers to housing placement.
• Lead and facilitate the CAHPS Steering Committee, including agenda development, coordination of partner participation, and tracking progress on Coordinated Entry system improvements.
• Participate in CAHPS-related meetings, including but not limited to the CAHPS Steering Committee, external stakeholders, and other CoC meetings as needed.
• Serve as the primary operational contact for Coordinated Entry partners across the CoC, managing relationships with CAHPS Liaisons, supporting partner coordination, communication, and data quality across participating agencies, and responding to the Coordinated Entry Helpdesk tickets.
• Build and maintain collaborative relationships with local government agencies, housing providers, victim service providers, healthcare organization, and nonprofit partner to strengthen CAHPS participation and regional system alignment.

• Systems Monitoring & Improvement
• Analyze CAHPS data using an equity lens to identify areas for system improvement.
• Ensure all Coordinated Entry phases are implemented consistently across participating agencies in accordance with HUD Coordinated Entry requirements and CoC policies.
• Provide performance monitoring and continuous quality improvement efforts to ensure a strong and responsive Coordinated Entry System.
• Monitor housing referrals made through the CAHP system and support collaborative efforts to remove barriers to housing for households on the by-name list; problem-solve areas for improvement with CAHPS partners in referral processes and outcomes.
• Ensure CAHPS program compliance with data privacy and confidentiality standards, including protections necessary for victims of domestic violence.
