Job Summary
The Intensive Case Manager provides home- and community-based services for clients residing in off-site housing. The role develops and implements an effective collaborative case plan that meets client needs and ensures housing stability. The manager assists clients through counseling/treatment, links them to appropriate services, and follows up to provide support and assure compliance.
Essential Duties and Primary Responsibilities- Coordinates client assessments, arranging clinical evaluations, psychiatric consultations, psychological testing, and functional assessments.
- Integrates and maintains a collaborative case plan for each client, emphasizing stable housing.
- Monitors implementation of the case plan by making regular home visits, linking clients with necessary providers, and maintaining contact throughout treatment.
- Manages community resources and engages clients to promote success with their case plan.
- Interfaces with outside agencies for specialized services, promoting coordinated care.
- Anticipates client needs for specific programs, initiates referrals, and follows up to ensure compliance.
- Assists clients with fiscal management, budget planning, and applications to entitlements.
- Assists clients in locating suitable housing, interfaces with landlords, completes move‑in paperwork per HUD regulations, and supports relocation.
- Maintains an independent caseload of clients.
- Participates in multidisciplinary meetings, training, and required meetings.
- Documents activities accurately and timely in required data systems, keeping records in accordance with HUD regulations.
- Communicates client information with supervisor and other professionals when consents are obtained.
- Generates activity reports, and participates in clinical outcome data collection as directed by supervisory staff.
- Maintains assigned client charts per program policies and facilitates community resource availability (e.g., birth certificates, , social security).

Education/
Experience:
A Master’s Degree from an accredited school in an area of human service, or related field, with at least two years of related experience. A Master’s Degree in psychology, counseling, social work, or related field is preferred, especially for those working toward professional licensure. Years of experience may be used to substitute for formal education. A valid Florida driver’s license is required.
Environment
The Intensive Case Manager operates in Broward Partnership settings, including offices, classrooms, group rooms, and other program areas both on and off campus. The position may involve active community engagement and representation of Broward Partnership.
