Job Title

Patient Coordinator

Job Summary

Perform clerical duties utilizing specific knowledge of medical terminology, PACE regulations, contract providers, and authorization procedures. Duties include scheduling appointments for participants, including coordination with caregivers, ALF’s, escorts, and transportation. Pull reports from the information system to monitor compliance. Oversee scanning of assessments by the medical receptionist into the software system.

Essential Job Functions
  • Schedules all participant diagnostic appointments and medical consultations for PACE participants and enters appointments into the PACE medical authorization system.
  • Tracks appointments and produces reports as requested to track data.
  • Coordinates with caregivers, transportation department, ALF staff, and escort, if needed, to assure that participants attend appointments.
  • Oversees unit‑specific documentation as per department guidelines and regulatory standards.
  • Provides necessary coordination with other departments.
  • Acts as support for the receptionist.
  • Performs other support functions as needed.
  • Attends department meetings and in‑services as needed.
Education and

Minimum Qualifications
  • High School Diploma or equivalent.
Experience
  • Two years office/clerical experience preferred, preferably in a healthcare setting.
Licenses/Certifications
  • N/A
Abilities Required
  • Bilingual (Spanish/English) preferred.
  • Knowledge of medical terminology.
  • Ability to work with geriatric participants.
  • Ability to gather data, compile information, and prepare reports.
  • Must be able to speak, read, and write English.
Environment / Work Conditions
  • Normal working conditions with adequate lighting and ventilation.
Infectious Material Exposure
  • No routine exposure to blood/body fluids/tissues, but may occasionally require performing unplanned activities which may involve such exposure.
