Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide Continuous Care to patients in the Broward County area on the following schedule: 7pm - 7:30am and every other weekend. What you'll do

As a Continuous Care LPN, you will play a vital role in supporting patients and families during some of the most critical moments in care.

• Deliver compassionate, high-quality care aligned with Trustbridge Hospice's mission, vision, and values
• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
• Coordinate and provide individualized care plans that prioritize comfort and dignity
• Utilize monitoring, intervention, and teaching skills to enhance quality of life
• Provide emotional support and reassurance to patients and families during overnight care
• Ensure exceptional, personalized service and a positive experience for every patient and family
• Maintain a strong commitment to patient satisfaction and overall care excellence

What you'll need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements preferred.
• Hospice Certification preferred.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
