* Position Summary:
*
* The Advanced Practice Provider (APP) will support oncology services with a dual focus on infusion center coverage and comprehensive survivorship care. This role is responsible for managing acute patient needs in the infusion setting while also leading survivorship care planning, monitoring, and coordination following completion of active cancer treatment.

* Key Responsibilities:
*
* + Conduct comprehensivesurvivorshipassessments including history, physical examinations,and interpretation of diagnostic studies.
+ Develop,implement and regularly update individualized survivorship care plans following completion of cancer treatment.
+ Summarize treatment history (including surgery, chemotherapy, radiation, andimmunotherapy) andoutline follow-up care
+ Coordinate transition from active oncology care to long-term survivorship.
+ Monitor for cancer recurrence and secondary malignancies
+ Oder and interpret appropriate surveillancelabs, imaging, and diagnostic tests.
+ Identify and managelate and long-term effects ofcancer and its treatment, including referral to rehabilitation, specialty services, or supportive therapies as needed.
+ Screen patients for psychosocial or emotional impacts and provide counseling and referrals as needed.
+ Counsel survivorson health promotion, disease prevention, and age/riskappropriate screeningsand vaccinations
+ Provide patient andcaregivereducation.
+ Maintain compliance with organizational policies, regulatory guidelines, and best practice standards.
+ Provide clinical support within the infusion center, including:
+ Supportive care and symptom management
+ Emergency management of infusion reactions
+ Evaluation and management of unexpected findings unrelated to infusion therapy, for example, possible DVT.

* Qualifications:
*
* Graduate of an accredited Physician Assistant or Nurse Practitioner Program
National Advanced Practice Registered Nurse Certification (APRN) OR national board certification as a Physician Assistant.
Current Physician Assistant or APRN license in the State of Florida.
AOCNP Certification
BLS/ACLS
** Preferred
