PART-TIME / PRN REGISTERED NURSE – ADULT & GERIATRIC CARE MANAGEMENT
We are seeking an experienced, compassionate Registered Nurse (RN) to join a growing adult and geriatric care-management practice on a part-time / PRN (as-needed) basis
.
This is not a traditional bedside nursing position. We are looking for an RN who enjoys working independently, advocating for older adults, solving problems, and helping families navigate an increasingly complicated healthcare system.
What You’ll Do- Conduct in-home and community-based client assessments
- Identify changes in condition, safety concerns and care needs
- Assist in developing and updating individualized care plans
- Coordinate with physicians, home health, hospice, rehabilitation, assisted living and other healthcare providers
- Attend medical appointments when needed and communicate recommendations to families
- Assist with hospital and rehabilitation discharge planning
- Review medications and identify potential concerns requiring physician/pharmacy follow-up
- Provide ongoing wellness visits and clinical oversight
- Communicate clearly with adult children and responsible parties regarding their loved one's condition and needs
- Help families understand appropriate levels of care and available resources
- Document assessments, visits and recommendations
- Active Florida RN license in good standing
- Experience working with older adults strongly preferred
- Background in geriatrics, home health, hospice, hospital case management, rehabilitation, assisted living or skilled nursing is highly desirable
- Strong assessment and documentation skills
- Excellent communication skills with families and healthcare professionals
- Comfortable working independently in the community
- Reliable transportation and ability to travel throughout Broward County
- Compassionate, resourceful and solution-oriented
Experience with
dementia, care management, transitions of care and senior living is a major plus.
This is a flexible, part-time/PRN position ideal for an experienced RN who may already be working part-time, semi-retired, raising a family, or simply looking for meaningful work outside of traditional nursing shifts.
Hours will initially be based on client need with the opportunity to grow as the care-management practice expands.
