The Palms at Plantation is looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our resident care team in Plantation, Florida.

As an LPN at The Palms at Plantation, you'll play an important role in supporting the health, safety, and well-being of our residents while helping them maintain their independence and quality of life.

• Provide direct nursing and resident care services according to each resident's individualized plan of care.
• Administer medications in accordance with standards of practice, Florida regulations, and community policies and procedures.
• Complete all required medication documentation, reporting, and follow-up.
• Perform resident assessments as required by state regulations and community policy under the direction of nursing leadership.
• Monitor residents for changes in physical, emotional, or psychosocial condition and immediately report concerns to the appropriate supervisor.
• Document relevant observations, clinical procedures, resident care, and communication with residents' responsible parties by the end of each shift.
• Assist with the development and maintenance of individualized resident care plans.
• Communicate effectively with residents, families, physicians, pharmacies, and other members of the care team.
• Provide guidance and support to caregivers and other resident care associates as appropriate.
• Promote resident dignity, independence, safety, and quality of life.
• Follow infection control, safety, emergency preparedness, and resident care procedures.
• Maintain a positive, professional, and team-focused approach to resident care.
• Perform other duties as assigned.

• Current Florida Licensed Practical Nurse (LPN) license in good standing required.
• High school diploma or equivalent (GED) required.
• Current CPR certification required.
• At least 1 year of related nursing experience preferred.
• Senior living, assisted living, memory care, or long-term care experience is a plus.
• Strong communication, organizational, and clinical skills.
• Compassionate and resident-focused approach to care.
• Dependable and able to work effectively both independently and as part of a team.
• Ability to meet the physical requirements of the position, including assisting with resident transfers and repositioning using appropriate techniques and equipment.
