Housing Case Manager
The Housing Case Manager is responsible for assessing and coordinating Housing Opportunities for Persons with AIDS (HOPWA) and other programs for Care Resource clients. Essential job responsibilities include HOPWA and other housing opportunities coordination, caseload management, service planning and documentation, managing resources, community involvement, safety, culture of service, contact responsibility, and physical requirements. The position requires skills in analysis, calculating, caring, checking for accuracy, collaborating, comforting, commitment, communication, compassion, conceptual thinking, conflict resolution, constructive criticism, continuous learning, cooperation, coordination, courteousness, critical thinking, customer service, documentation, financial analysis, integrity, interpersonal skills, Microsoft Office, multitasking, patience, planning and organizing, problem solving, quick thinking, self-motivation, telephone, verbal communication, written communication, and healthcare systems.
Preferred skills include bilingual proficiency in English-Spanish or English-Creole. Required education is a bachelor's degree or better in public administration or related field. Preferred education includes a bachelor's degree in health administration or social work. Required experience includes two years in homeless/social services. Preferred experience includes one year in HIV/AIDS.
