Moms In Motion is seeking a Service Facilitator (SF) Case Manager to visit families at their homes and help them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT). You’ll support independent, safe living for clients and work remotely from a laptop when not on assignments.
We value compassion, organization, and community focus. A valid driver’s license is required and 2+ years of relevant experience is preferred; travel to VA is expected.
