Bridge Senior Living -  - Responsibilities:
Provide direct resident care services including medicine distribution;
Follow individualized plan of care for each resident;
Observe residents for changes in condition and report any changes immediately to the supervisor and/or the Wellness Director;
Document all pertinent/relevant observations, red flags, incidents, interventions, communication with the Resident’s responsible party, physician (as necessary), clinical procedures, and daily medication assistance/care throughout the shift;
Complete and maintain the current individualized care plan for each Resident
