Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• 1-2 years of experience as a Registered Nurse in a clinical care setting
• 2+ years of home health experience
• CHPN certification (preferred)
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance

Roles & Responsibilities:

• Provide skilled nursing care to patients in their homes, assessing and addressing their physical, psychosocial, and environmental needs.
• Develop, document, and evaluate individual Plans of Care to ensure quality and continuity of care.
• Educate patients and their families on care, medications, and safety practices, promoting self-care.
• Supervise and provide clinical direction to support staff to ensure quality service delivery.
• Monitor assigned cases to ensure compliance with regulatory and payer requirements.
• Maintain effective communication with patients, families, and healthcare professionals.

Compliance / Notes:

• Must maintain current licensure and meet mandatory continuing education requirements.
• Ability to work a flexible schedule and travel locally is required.
• Must maintain confidentiality and demonstrate effective teamwork.
