PRN Hospice LPN

Role Overview

The PRN Hospice LPN provides nursing services in homecare or facility settings, following state nursing board guidelines. They assess patient needs, participate in care planning, and deliver nursing care under the supervision of a registered nurse. The LPN collaborates with the interdisciplinary team, communicates effectively with patients and families, and ensures proper documentation. They support patient independence, participate in education and counseling, and attend team meetings to coordinate care.

• Assess physical and mental health; document findings.
• Implement and communicate the Plan of Care as delegated by a registered nurse or authorized professional.
• Facilitate patient and caregiver education, addressing questions and needs.
• Participate in interdisciplinary team conferences and referrals.
• Maintain accurate clinical documentation and adhere to infection control practices.
• Ensure professional development, licensure compliance, and effective time management.

• Diploma from an approved nursing program; current LPN license in a multistate compact.
• CPR certification; CHPN preferred.
• Minimum 1-year experience in hospice, palliative care, or related fields; proficiency in computer use.
