Description:
Provides, coordinates, and directs the provision of home nursing care according to physician s orders.
Care is based on agency policies and procedures through the competent application of the nursing process.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Home visits are required.
Periodic office meetings are necessary.
Contractor will be assigned between 5-10 patients per day.
Types of cases include Start of Care, Wound Care, and Follow-Up Visits.
Must be willing to travel up to 75 miles from the home clinic.
Mileage will be reimbursed at the IRS standard rate.
All documentation must be completed within 24 hours of the patient appointment.
Delayed or incomplete documentation may impact billing and follow-up patient care.
There are no guaranteed hours for this position.
Weekly productivity expectations are set at 30 points.
All overtime must be pre-approved.
Clinicians must be open to accepting every patient on their schedule, regardless of distance.
Clinical staff must ensure that they can consistently meet the demands of the position.
