Registered Nurse

Communi Care is seeking a compassionate Registered Nurse to provide high-quality, resident-centered care in our skilled nursing and long-term care setting. As part of our collaborative, learning-focused team, you will assess and monitor residents, administer medications and treatments, coordinate care plans, and communicate with families and healthcare providers. You will mentor caregivers, support quality and safety initiatives, and document care accurately.

Responsibilities

• Provide direct resident-centered nursing care in a skilled nursing and long-term care environment
• Assess residents' conditions, monitor changes, and document findings accurately in the EMRAdminister medications and treatments safely in accordance with physician orders and facility policies
• Develop, update, and implement individualized care plans in collaboration with the interdisciplinary team
• Communicate effectively with residents, families, and healthcare providers to support care goals
• Support and mentor CNAs and other caregivers to ensure consistent, high-quality care
• Participate in quality improvement, safety, and infection control initiatives
• Respond to emergencies, perform basic life support, and escalate clinical concerns as needed
• Educate residents and families on conditions, medications, and care plans
• Contribute to a compassionate, team-focused culture aligned with Communi
