- Deliver quality nursing care to patients in accordance with state regulations and company standards.

Position in Akron, Colorado. Direct patient care on a 3×12 night shift schedule (6:00 PM–6:00 AM). 13‑week assignment with travel assistance and housing.
Key Responsibilities- Provide direct nursing care under the supervision of an RN or physician.

Focused on hospital avoidance, monitoring patients via RPM dashboards, contacting patients, and coordinating follow‑ups.
Job Responsibilities- Proactively manage patient health by monitoring remote patient monitoring dashboards and responding to vital sign alerts.
- Conduct regular outreach calls to patients to assess symptoms and encourage consistent daily device usage.
- Coordinate timely follow‑up care with Nurse Practitioners, Primary Care Providers, or patient caregivers.
- Maintain accurate documentation of all patient encounters within the EMR and internal tracking systems.
- Ensure patient compliance and troubleshoot remote monitoring device issues.
- Act as the primary clinical point of contact for an assigned panel of remote care patients.
- Assist with patient enrollment and onboarding activities as needed to support growth.
- Collaborate daily with the clinical team and support personnel.
- Active LPN license in Virginia.
- Strong clinical judgment, verbal communication, and phone skills.
- Highly organized, proficient with technology, and adept at managing simultaneous digital platforms.
- Prior experience in Remote Patient Monitoring or telehealth desirable.
- Deep commitment to preventive care and reducing hospital readmissions/ER utilization.
