We are a private, multi‑location healthcare provider looking for a Licensed Practical Nurse (LPN) to join our interdisciplinary team at Fort Morgan. • Deliver quality nursing care to patients in accordance with state regulations and company standards.

Position in Akron, Colorado. Direct patient care on a 3×12 night shift schedule (6:00 PM–6:00 AM). 13‑week assignment with travel assistance and housing.

Focused on hospital avoidance, monitoring patients via RPM dashboards, contacting patients, and coordinating follow‑ups.

• Proactively manage patient health by monitoring remote patient monitoring dashboards and responding to vital sign alerts.
• Conduct regular outreach calls to patients to assess symptoms and encourage consistent daily device usage.
• Coordinate timely follow‑up care with Nurse Practitioners, Primary Care Providers, or patient caregivers.
• Maintain accurate documentation of all patient encounters within the EMR and internal tracking systems.
• Ensure patient compliance and troubleshoot remote monitoring device issues.
• Act as the primary clinical point of contact for an assigned panel of remote care patients.
• Assist with patient enrollment and onboarding activities as needed to support growth.
• Collaborate daily with the clinical team and support personnel.

• Active LPN license in Virginia.
• Strong clinical judgment, verbal communication, and phone skills.
• Highly organized, proficient with technology, and adept at managing simultaneous digital platforms.
• Prior experience in Remote Patient Monitoring or telehealth desirable.
