Licensed Practical Nurse – General Care

We are a private, multi‑location healthcare provider looking for a Licensed Practical Nurse (LPN) to join our interdisciplinary team at Fort Morgan. Licensed Practical Nurse – 13‑Week Travel Assignment (Night Shift)

Position in Akron, Colorado. Direct patient care on a 3×12 night shift schedule (6:00 PM–6:00 AM). 13‑week assignment with travel assistance and housing.

Licensed Practical Nurse – Remote Patient Monitoring

Focused on hospital avoidance, monitoring patients via RPM dashboards, contacting patients, and coordinating follow‑ups.

Job Responsibilities
  • Proactively manage patient health by monitoring remote patient monitoring dashboards and responding to vital sign alerts.
  • Conduct regular outreach calls to patients to assess symptoms and encourage consistent daily device usage.
  • Coordinate timely follow‑up care with Nurse Practitioners, Primary Care Providers, or patient caregivers.
  • Maintain accurate documentation of all patient encounters within the EMR and internal tracking systems.
  • Ensure patient compliance and troubleshoot remote monitoring device issues.
  • Act as the primary clinical point of contact for an assigned panel of remote care patients.
