LPN Licensed Practical Nurse

Life Care Centers is seeking a compassionate LPN Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you'll provide direct, patient-first nursing care to seniors in a rehabilitation and long-term care setting. Responsibilities include administering medications, monitoring vital signs, assisting with treatments, and accurately documenting care. You'll collaborate with an interdisciplinary team to support individualized care plans, educate residents and families, and promote excellent healthcare outcomes in a family-focused environment that encourages professional growth.

Responsibilities

• Provide direct nursing care to residents in a senior living and rehabilitation setting
• Administer medications and treatments according to physicians' orders and facility policies
• Monitor, document, and report changes in resident condition, including vital signs and symptoms
• Support development and implementation of individualized care plans with the interdisciplinary team
• Maintain accurate, timely documentation in electronic health records
• Promote patient safety and infection control standards at all times
• Assist residents with daily living activities as needed, ensuring dignity and comfort
• Educate residents and families on care plans, medications, and health maintenance
• Coordinate with RNs, physicians, therapists, and other team members for optimal outcomes
