Page Health & Rehab Center is a premier skilled nursing and rehabilitation facility located in Fort Myers, Florida. Our in-house subacute rehabilitation team is dedicated to providing high-quality, patient-centered care that supports recovery, mobility, and improved quality of life.
We are currently seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our collaborative rehab team.

Physical Therapist Position SummaryThe Physical Therapist evaluates residents’ physical abilities, develops individualized treatment plans, and delivers evidence-based therapeutic interventions to restore mobility, strength, balance, and functional independence. This role works closely with an interdisciplinary team to ensure coordinated and effective patient care.
Physical TherapistKey Responsibilities
- Evaluate residents’ mobility, strength, balance, endurance, and functional abilities
- Develop and implement individualized physical therapy treatment plans
- Provide high-quality, evidence-based PT services aligned with patient goals
- Collaborate with Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and other interdisciplinary team members
- Educate residents, families, and caregivers on treatment plans, mobility techniques, and safety strategies
- Maintain accurate and timely clinical documentation in compliance with facility and regulatory standards
- Participate in care planning meetings, interdisciplinary team conferences, and discharge planning
- Adhere to all state, federal, and facility policies, including HIPAA and infection control guidelines
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in the state of Florida
- CPR certification (or willingness to obtain upon hire)
- Experience in skilled nursing or subacute rehabilitation preferred, but not required
- Strong communication, organizational, and interpersonal skills
- Ability to work independently and collaboratively within a multidisciplinary team
