Minimum to Midpoint Pay Rate: $51.54 - $67.01 / hour

Complex Care Center is a transition of care clinic for adults managing multiple or chronic medical conditions, with the goal of enhancing quality of life, reduce hospital readmissions, and supporting patients from hospital discharge to a primary care or specialty provider clinic. The clinic is open seven days a week, from 7am-7pm, with every other weekend coverage.

CCC pharmacists are responsible for a wide range of distributive and clinical pharmacy services including:
Pharmacist managed warfarin clinic; infusion medications for the clinic use including IVIG, iron therapy and a wide range of specialty medications; parenteral antibiotics; dose optimization through Therapeutic drug monitoring services; wound care related medication; patient education, and renal dose adjustment.

Education: Pharm

D required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP).

Experience: Five years of professionally responsible pharmacist experience required for Bachelors degree holders. One year of recent inpatient hospital experience as staff pharmacist or pharmacy intern in a general medical/surgical hospital preferred for Pharm

D candidates.

Certification: BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire). ACLS (AHA) and/or PALS (AHA), depending on practice site.

License: State of Florida Department of Professional Regulation, Registered Pharmacist Licensure.

Other: Strong communication and interpersonal skills, data entry skills, analytical and basic math skills required.
