Fort Myers Rehabilitation and Nursing Center is seeking a dedicated and compassionate Per Diem Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist will evaluate, plan, and implement individualized treatment programs for residents to improve mobility, reduce pain, restore function, and promote overall wellness. This per diem position offers flexible scheduling and the opportunity to work with a collaborative interdisciplinary team in a skilled nursing and rehabilitation setting.

• Evaluate residents upon admission and develop individualized treatment plans.
• Provide physical therapy services in accordance with physician orders and established care plans.
• Implement therapeutic exercises, gait training, balance activities, transfer training, and modalities as appropriate.
• Monitor and document patient progress accurately and timely in compliance with facility, state, and federal guidelines.
• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other team members.
• Educate residents and families on therapy goals, home exercise programs, and safety techniques.
• Ensure compliance with all regulatory standards and facility policies.

• Graduate of an accredited Physical Therapy program.
• Current and active Florida Physical Therapist license.
• Current CPR certification.
• Experience in a skilled nursing facility or rehabilitation setting preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
