Job Description:
Complete Care Home Health Agency, Inc. is seeking a compassionate and dependable Registered Nurse (RN) to provide intermittent skilled nursing services to Medicare and Medicaid patients in the Port Charlotte and Surrounding areas, Florida area. The RN will provide high-quality home health care in accordance with physician orders, Medicare Conditions of Participation, Medicaid guidelines, and agency policies.
- Provide skilled nursing visits to Medicare and Medicaid patients in their homes.
- Perform patient assessments and develop individualized plans of care.
- Administer medications, injections, wound care, catheter care, and other skilled nursing services.
- Complete OASIS assessments, recertifications, resumptions of care, transfers, and discharge documentation as required.
- Educate patients and caregivers on medications, disease management, safety, and treatment plans.
- Monitor patient progress and report significant changes to physicians and supervising clinicians.
- Coordinate care with therapists, home health aides, physicians, case managers, and caregivers.
- Complete accurate and timely electronic documentation in Well Sky/Kinnser.
- Maintain compliance with Medicare, Medicaid, AHCA, HIPAA, and agency standards.
- Participate in case conferences and quality assurance activities as needed.
- Active Florida Registered Nurse (RN) license.
- Minimum one (1) year nursing experience preferred.
- Home health experience preferred.
- OASIS experience preferred.
- Current BLS/CPR certification.
- Valid Florida driver’s license and reliable transportation.
- Ability to work independently and manage multiple patient visits efficiently.
