Minimum to Midpoint Pay Rate: $28.83 - $38.92 / hour

Lee Physician Group is seeking an experienced RN to join their Virtual Health Team. This RN will need to have exceptional triage and telephone communication skills.

• 1 year of acute care RN experience is required
• 1 year of cardiac experience preferred
• 1 year of home health experience preferred

Coordinates telemonitoring systems and processes for Lee Health to improve patient outcomes and reduce readmissions to the hospital. Demonstrates exceptional triage skills and appropriate judgment/assessment skills to be able to make independent clinical decisions in monitoring patients' vital signs and conduct appropriate follow up as indicated. Has ability to deal with physicians, clinicians, and patients. Possesses computer skills in order to be proficient in all aspects of the monitoring station and runs various reports on a regular basis to support quality initiatives and ensure positive outcomes in patient care.

Performs in home visits as needed or assigned by the supervisor.

Graduate of an accredited school of professional nursing.

• 1 Year Acute Care – Required
• 1 Year Cardiac – Preferred
• 1 Year Home Health – Preferred

• Registered Nurse License – Required

Currently licensed or eligible for RN licensure in the State of Florida.

• BLS (American Heart Association / Basic Life Support) – Required
