Summary
The Home Therapy Registered Nurse -- Traveler provides patient training and ongoing support for home dialysis modalities across U.S. Renal Care locations. This position offers nursing relief/support for assigned programs on a temporary or short-term basis.
Responsibilities- Participate in and support corporate initiatives that promote improved care and growth of the home department.
- Understand vendor product delivery methods and associated fees; teach patients the importance of proper and timely order placement and ensure supplies are delivered in a timely manner to avoid care disruption.
- Assist with clinic and patient supply inventory; ensure usage aligns with prescriptions and place orders promptly to avoid fees and penalties.
- May assist with equipment management including equipment tracking and retrieval as needed.
- Perform duties as assigned to meet patient care or operational needs of the program.
- Assist with improving patient outcomes through tracking, trending, critical thinking, and root cause analysis; support data collection for continuous quality improvement activities.
- Obtain routine and non-routine laboratory tests as ordered; communicate critical lab values and urgent patient needs to the physician or extender promptly.
- May complete and document monthly reviews of patient medication profiles; administer medications as ordered by the physician.
- Provide patient education and follow up as needed; assist with program target goals for patient outcomes in line with quality care and company goals.
- Know and comply with federal, state, local laws and regulations, including practice act requirements relevant to home dialysis and patient care.
- Perform duties within policy and procedures and in accordance with applicable laws and regulations.
- Train patients (and/or care partners) in self-care peritoneal dialysis or hemodialysis as per RN qualifications; may conduct home visits to assess home environment as needed.
- Inform direct supervisor and program administrator of incidents, conditions, and concerns related to patient care and safety; document nursing services in the Electronic Medical Record with accuracy to reflect patient status and interventions.
- Assist with developing and implementing patient plans of care with the interdisciplinary team; participate in infection control monitoring and reporting.
- Use personal protective equipment as necessary; be familiar with emergency equipment and procedures; communicate and review emergency preparedness with home patients.
- Communicate the on-call system to patients and ensure access to nursing support at all times; may perform on-call nursing services nights and weekends as needed.
- Flexibility with staffing locations and hours to accommodate patient and program needs; regular and reliable attendance is required.
- Home Hemodialysis Only: coordinate home evaluation with technical services to ensure electrical, plumbing, and drainage requirements are met prior to patient acceptance; assist with water sampling per policy and monitor results.
- Communicate patient assessment results to physicians, team members, and others as appropriate.
- Maintain positive collaborative relationships with physicians, state agencies, and the community.
- Promote guest customer service standards and develop effective relationships at all levels of the organization; respond effectively to inquiries or concerns.
- Participate in staff meetings and continuing education offerings in compliance with policy; may delegate tasks to licensed and unlicensed staff per state practice acts.
- Assist with staff training as requested; lead staff in team concepts and promote teamwork.
- Current RN license in multiple states; licenses must be maintained in good standing. Obtain applicable state license based on assignment within required time frames.
- 12 months or more of nursing experience; 6 months or more of home modality experience required.
- Flexibility with schedule and willingness to travel to assignment locations throughout the U.S. and Guam.
- CPR certification prior to patient care assignment.
- Ability to distinguish all primary colors.
- Must complete the Home Therapy Nurse Orientation program and maintain annual competency for the assigned modality.
- Working knowledge of English and ability to communicate verbally and in writing.
- Basic computer skills; proficiency in USRC clinical applications within 90 days of hire.
- Must meet state-specific practice requirements and any additional background clearances or orientation requirements for the program.
