Job Overview
The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit.
Responsibilities- Provide direct care to patients and families as part of the IDG, incorporating psychosocial, spiritual, cultural, physical, and biological components.
- Coordinate individualized Plans of Care that accurately reflect evolving patient needs.
- Educate patients, families, caregivers, and other health professionals on disease processes, palliative interventions, caregiving, and safety practices.
- Report changes in patient condition to relevant members of the IDG or other health professionals.
- Participate with the IDG to evaluate hospice referrals and admissions for appropriate level of care.
- Attend patient deaths, prepare the body for transport, and oversee disposal of medications.
- Provide bereavement interventions to family and caregivers in collaboration with psychosocial staff.
- Present concise and pertinent oral and written reports to the IDG and encourage input from all disciplines.
- Communicate accurately and completely with physicians, staff, patients, families, and supervisors.
- Supervise patient care provided by other nursing team members (e.g., LPNs, Hospice Aides, contract nurses).
- Be available for emergency work, such as reporting to designated locations during lockdowns or emergencies.
- Perform other duties as assigned.
- Current RN license in the state of employment.
- Active BLS certification for health care professionals.
- Minimum one (1) year of nursing experience; hospice or hospital experience preferred.
- Experience with EMR/EHR systems.
- Valid driver’s license and automobile insurance per company policy; reliable transportation to meet visit schedule.
- Ability to use equipment with visual and auditory mechanisms and communicate effectively in English.
- Flexible schedule to meet the needs of Hospice House patients.
- Capacity to manage the emotional stress of caring for terminally ill patients and their families.
- Physical ability to access patient rooms, other areas of the Hospice House/Hospital/ALF, and homes (including navigating stairs, narrow spaces, heat, and lack of air conditioning).
- Physical capability to perform essential functions such as lifting patients, bending, pushing/pulling, and kneeling.
- For PACE employees: completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs.
- Preparedness for cross‑training to hospice house or field positions when required.
- Ability to complete clinical admissions for all company services, including explaining services, addressing concerns, obtaining consents, initiating care plans, and documenting orders.
- Pay rate: $45.00 per hour.
- Position requires conditional offer drug and/or alcohol testing and ongoing compliance with the Drug‑Free Workplace Policy.
- Background screening required through the Florida Care Provider Background Screening Clearinghouse after a conditional offer and ongoing eligibility.
Typical duties require seeing, hearing, talking, walking, using hands, standing, sitting, reaching, lifting, bending, balancing, pulling, pushing, stooping/crouching, kneeling, and climbing stairs.
