Role Overview

The Licensed Practical Nurse (LPN) provides compassionate, high-quality nursing care to seniors in a community setting, supporting residents' safety, dignity, and well-being.

• Deliver individualized nursing care per care plans and policies.
• Assess residents' physical and psychosocial conditions, monitor changes, and document accurately.
• Administer medications and treatments as prescribed, collaborating with healthcare teams.
• Coordinate care with physicians, families, and other providers.
• Support health promotion, disease prevention, and wellness initiatives.
• Respond to emergencies, incidents, and resident concerns with sound clinical judgment.
• Assist with care planning, resident evaluations, and staff training.
• Ensure regulatory compliance and participate in quality assurance activities.

• Current LPN license or equivalent as per state regulations.
• Minimum two years of nursing experience, preferably in assisted living or long-term care.
• Strong clinical judgment, problem-solving, and decision-making skills.
• Effective organizational, communication, and time management abilities.
• Ability to manage multiple priorities, delegate, and respond to emergencies.
• Proficiency with electronic documentation and basic computer skills.
