Position SummaryThe Clinical Manager is responsible for overseeing day-to-day clinical operations, supporting field staff, and ensuring patient care is delivered safely, effectively, and in compliance with physician orders, agency policies, and regulatory requirements.
This role provides direct support to clinical staff, assists with case management oversight, and works closely with leadership to maintain quality, efficiency, and continuity of care.
This position reports directly to the Director of Nursing (DON) and partners closely with the Administrator and interdisciplinary leadership team.
The Clinical Manager provides supervision, guidance, and clinical support to field clinicians, including:
- Registered Nurses (RNs)
- Licensed Practical Nurses (LPNs)
- Certified Nursing Assistants (CNAs)
- Therapy staff, as assigned
This role is clinical and administrative in nature and functions within the scope of Florida RN licensure. The Clinical Manager supports patient care coordination, staff supervision, and clinical oversight but does not replace the responsibilities of the Director of Nursing.
Clinical Leadership & Oversight- Supervise and support field clinicians in delivering high-quality patient care.
- Ensure care is provided in accordance with physician orders and individualized care plans.
- Provide clinical guidance and assist with complex patient care concerns.
- Monitor patient outcomes while promoting quality, safety, and regulatory compliance.
- Support interdisciplinary collaboration to ensure continuity of care.
- Partner with the staffing team to ensure appropriate clinician coverage.
- Assist with clinician assignments based on patient needs, clinical expertise, and geographic location.
- Address scheduling challenges, staffing concerns, and field support needs.
- Respond promptly to clinical escalations and patient care issues.
- Promote compliance with Medicare Conditions of Participation, Florida regulations, and agency policies.
- Review clinical documentation for accuracy, completeness, and timeliness.
- Participate in quality improvement initiatives and performance improvement plans.
- Assist with audits, surveys, accreditation activities, and regulatory inspections.
- Serve as a clinical resource for field staff, patients, families, and referral partners.
- Communicate changes in patient condition and care plans with physicians and interdisciplinary team members.
- Collaborate with Intake, Scheduling, Therapy, Billing, and Leadership to ensure seamless patient care.
- Assist with onboarding and orientation of new clinical employees.
- Coach, mentor, and support clinicians through ongoing education and performance feedback.
- Participate in competency evaluations, clinical training, and in-service education.
- Foster a culture of accountability, teamwork, professionalism, and continuous improvement.
- Bachelor’s degree in nursing (BSN required; MSN preferred) or related healthcare field required. Current Registered Nurse (RN) license in good standing required. Minimum of three (3) years of clinical nursing experience required, with at least two (2) years in home health and one (1) year in a supervisory or leadership role required.
- Strong knowledge of home health regulations and Medicare Conditions of Participation
- Experience with EMR systems such as Axxess, Well Sky, (Kinnser), or similar preferred
- Current CPR certification required
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to pass background screening and all compliance requirements
- Strong clinical judgment and leadership abilities
- Excellent communication and interpersonal skills
- Ability to manage multiple priorities in a fast-paced environment
- Strong problem-solving and decision-making skills
