Licensed Practical Nurse

Job Description

We are seeking a dedicated and skilled Licensed Practical Nurse to join our team in Fort Myers, FL. The Licensed Practical Nurse will manage the care of assigned residents in accordance with prescribed Active Treatment Plans and provides patient care and coordinates the services of a multidisciplinary team in collaboration with facility staff and personnel.

Responsibilities
  • Utilizes clinical nursing competency observing, monitoring, and providing care, treatment and services to residents.
  • Facilitates interdepartmental communication and problem solving by interfacing with all members of the healthcare team.
  • Perform primary nursing duties for assigned residents/shift to ensure daily, monthly, quarterly, and annual nursing summaries in accordance with residents' Active Treatment Plan.
  • Monitor physical health of resident, relaying safety and health concerns to the Director of Nursing, physician, pharmacist, and other disciplines as needed.
  • Implement administration of prescribed medications and treatments in accordance with State and clinical nursing standards.
  • Receive, transcribe, fax orders to pharmacy and enter orders in the electronic health record.
  • Documents timely, outcome based information, facilitating interdisciplinary communication.
  • Participate and attend facility meetings and in-services to ensure resident health care needs are maintained.
Required Qualifications
  • Current license in the state of Florida.
  • No experience required.
Preferred Qualifications

Education:

High School Diploma/GED preferred.

Experience:

At least six months hospital or rehabilitation nursing experience preferred.
