ARNP Community Resource Coordinator

Eckerd Connects seeks an ARNP Community Resource Coordinator to support justice-involved youth by integrating medical insight with community-based services. The ARNP will assess health-related needs, coordinate care plans, and link youth and families with medical, behavioral health, and social service resources. Responsibilities include collaborating with DJJ partners, facilitating referrals, monitoring outcomes, educating youth and caregivers on wellness, and maintaining accurate documentation.

This role works closely with multidisciplinary teams to reduce recidivism, improve health and stability, and advance Eckerd Connects' mission to strengthen vulnerable youth and families.
