Housing Opportunities For Persons With Aids (Hopwa) Case Manager

This position serves as a Housing Opportunities for Persons with AIDS (HOPWA) Case Manager, providing comprehensive housing-focused case management services to individuals and families living with Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/AIDS). This role involves assessing client needs, developing individualized service plans, and connecting clients to housing resources and social services to enhance their quality of life.

This position supports participants across multiple Housing Opportunities for Persons with AIDS (HOPWA) program components, including Short-Term Rent, Mortgage, and Utility Assistance (STRMU), Permanent Housing Placement (PHP), Transitional Housing, and Tenant-Based Rental Assistance (TBRA).

Duties and Responsibilities

Build Housing Stability

  • Meet with clients to complete intake, review documents, and determine program eligibility.
  • Complete assessments to identify client needs and available resources.
  • Develop housing stability plans and track client progress.
  • Maintain regular communication with clients and provide ongoing support.
  • Conduct home visits as needed.
  • Attend court hearings when required.
  • Provide guidance on behaviors and actions that support successful housing outcomes.

Connect Clients to Support

  • Connect clients with community resources, healthcare services, mental health support, and financial assistance programs.
  • Use case management strategies to help clients remain engaged in their goals.
  • Assist clients with navigating available services and systems.
  • Provide housing support services through HOPWA programs, including STRMU, PHP, Transitional Housing, and TBRA.

Work with Landlords and Partners

  • Maintain professional relationships with landlords, vendors, and community organizations.
  • Assist with resolving housing-related concerns between clients and landlords.
  • Review lease agreements and confirm compliance with program requirements.
  • Inspect properties when required and communicate repair needs.
  • Maintain accurate client records in HMIS.

Support Housing Success

  • Provide budgeting assistance, referrals, and housing retention support.
  • Coordinate with team members to promote successful housing outcomes.
Education, Experience, Licensing

Minimum Qualifications

Candidates must meet the following requirements:

  • Associate degree in human services, social work, psychology, or a related field; or
  • Two years of college-level coursework/training in human services, social work, or a related field; or
  • Equivalent relevant work experience that substitutes for education on a year-for-year basis;

And:

  • Two years of experience in social services, case management, care coordination, or a related field
  • Valid driver's license with a clean driving record

Additional Requirements (Grant-Funded Position)

In addition to the Lee County job classification minimum requirements listed above, this is a grant-funded position that has the following requirements per the standards set forth by the grant:
Bachelor's degree in Psychology, Social Work, Human Services, or a related field OR a Bachelor's degree in another discipline plus six months of case management experience.

Preferred Qualifications

  • Experience with housing programs or homelessness services
  • Experience with HOPWA programs
  • Knowledge of trauma-informed care, motivational interviewing, and supportive housing models
