Looking for a compassionate Case Manager with experience providing direct care to children experience serious emotional disturbances. Skilled in providing assessments and completing required documentation. Committed to providing resources and collaborative care in our Children's Case Management program.

• Provide case management services to children and youth in need of community based mental health services.
• Maintains a caseload of 20 patients and maintains case record documentation to meet DCF, Medicaid, and Salus Care policies and procedures.
• Conducts activities designed to procure specified services, treatment, and resources for patients.
• Assumes an advocacy role for patients to assure access to services in the service plan and monitor these services.
• Completes assessments and develops a service plan that reflects assessment outcomes.

• Successful pre-employment medical screening (drug test & TB)
• Completion of all required training

• Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and (1) one year of full time or equivalent experience working with children with serious emotional disturbances or have a Bachelor’s degree from an accredited university or college and (3) three years full time experience working with children with serious emotional disturbances.
• Must possess a valid Florida driver’s license and the appropriate level of automobile insurance coverage. Must have a reliable vehicle.
• Must be able to attain competency with company electronic health record and use it effectively within the required time frames.
• Training Requirements:
  Completed or agrees to complete AHCA-approved mental health targeted case management training within (3) three months of initially providing Medicaid services.
• Within (6) six months must obtain Certified Behavioral Health Case Manager Certification from Florida Certification Board.
