Hearth Hospice is seeking a Per Diem Hospice RN Case Manager to join our dedicated nursing team delivering hands‑on hospice care across the Fort Oglethorpe community. You’ll coordinate services, support patients and families, and work with the interdisciplinary care team to ensure comfort and dignity at every step.
The ideal candidate is a licensed RN with at least one year of medical‑surgical/acute care experience; hospice experience is a plus.
