Family Development Specialist

Join Health Connect America, Inc. as a Family Development Specialist supporting children, adolescents, and families through in home, in office, and community based services. You will assess family strengths and needs, develop individualized service plans, provide parent coaching, skill building, and crisis support, and coordinate with schools, agencies, and community partners. Collaborate with a multidisciplinary team to monitor progress, document services, and advocate for needed resources.

Responsibilities

• Conduct family-focused assessments to identify strengths, needs, and goals.
• Develop and implement individualized service or treatment plans.
• Provide in-home and community-based parent coaching and skills training.
• Offer crisis intervention and stabilization support as needed.
• Coordinate care with schools, courts, and community agencies.
• Advocate for families to access community and behavioral health resources.
• Maintain accurate, timely documentation and progress notes.
• Participate in multidisciplinary team meetings and case consultations.
• Monitor outcomes and adjust plans based on family progress.
• Engage families using trauma-informed, culturally responsive practices.
