Activa Home Health, a licensed, Medicare-certified agency, is seeking skilled and compassionate Registered Nurses to provide Medicare home health visits in both full-time and per-diem roles. This is an exciting opportunity, and we are looking for Nurses across multiple communities in Indian River, St. Lucie, and Martin Counties, delivering high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities
- Deliver skilled nursing care per the Plan of Care during Medicare home health visits
- Accurately document visits in the EMR, including OASIS assessments
- Communicate changes in patient condition promptly with the interdisciplinary care team
- Provide clinical services within scope of practice, including:
- Medication administration
- Wound care
- Infusion therapy
- Catheter care
- Safe transferring and positioning of patients
- Collaborate with Nurse Supervisors and Managers of Clinical Practice
- Demonstrate professionalism, empathy, attention to detail, and strong clinical judgment
- Active Florida Registered Nurse (RN) license
- Medicare home health experience required
- OASIS experience required
- Wound care experience required
- Proficiency with EMR systems (Well Sky Kinnser preferred)
- Minimum 1 year of home health experience
- Strong communication, teamwork, and patient-focused skills
