Minimum Bi-weekly Rate of Pay $1,588.86.(In accordance with current spending restrictions, an internal promotion into the position will be filled at the minimum of the pay grade or up to 5% of the employee's current rate, whichever is higher.)
DESCRIPTION:
This work is providing Prearrest Delinquency Citation (PDC) Case Manager for the circuit and is responsible for ensuring that the program area complies with all laws, rules and policies related to youth referrals and intake process.
This position may require flexibility with the work schedule.
DUTIES AND RESPONSIBILITIES:
This position serves as the Prearrest Delinquency Citation (PDC) Case Manager for the circuit and is responsible for ensuring that the program area complies with all laws, rules and policies related to youth referrals and intake process.
Attend all staff and departmental trainings and meetings as required. Complete a minimum of 24 hours of training annually. Perform other related duties as required.
Operate a variety of general office equipment such as: copy machine, typewriter, personal computer terminal, fax machine, etc.
Provide customer service to internal and external customers.
Experience using Microsoft programs to perform routine clerical and administrative duties.
This position must comply with the requirements set forth in Section 215.422 Florida Statutes, Prompt Payment Compliance Law.
Case Management, Referrals, and Assessment
Confirm youths' eligibility when receiving a referral from Florida Department of Juvenile Justice (FDJJ) and notify the referring law enforcement agency or state attorney's office if non-eligible or enter required data in the Juvenile Justice Information system (JJIS) Prevention Web if eligible.
Initiate contact with the youth's family upon eligibility to schedule intake assessments.
Conduct in person intake assessments to provide an overview of the program and complete the Prevention Assessment Tool (PAT).
Formulate a case plan using the appropriate Department approved instrument(s) that addresses the specific needs of the youth and family. Prior to the development of or revisions to the case plan staff should negotiate, implement, and modify the plan based on input from the youth and family, and the risk and needs identified by the instrument in accordance with departmental rule/policy.
Assist the youth and family with accessing community resources and services at an appropriate agency or placement resources, including intra-agency services, education and employment resources that will address youth and family needs to complete requirements of case plan.
Work with the youth to establish personal goals/ plans and provide ongoing support to help youth achieve their goals and maintain skills.
Discuss with youth and family projected time frames for face-to-face contact to review documentation provided by youth and family to determine if all requirements within the plan have been completed.
Case Management and Interventions
Review documents (e.g., community service hours, receipt(s) for restitution/fees, verification of participation in educational/CTE classes, verification of participation in counseling/treatment, etc.) as applicable to determine if the youth has satisfactorily completed all requirements of the program.
Follow up with supervisor to determine satisfaction in completing all requirements, and create an extended case plan with deadline dates, if requirements are not met.
