Aftercare Resource Navigator

Eckerd Connects seeks an Aftercare Resource Navigator to support justice-involved youth as they transition back to their communities. In this role, you will assess youth and family needs, develop individualized aftercare plans, and connect clients with community-based resources such as counseling, education, housing, and employment services. You will collaborate closely with juvenile justice partners, schools, and service providers, monitor progress, document outcomes, and advocate for youth and families.
